6.15.21 Letter To Families and Friends

ATTENTION ALL RESIDENTS / FAMILIES:

 

Please Note: Strict adherence to the guidelines is to be followed:

 

➢ Preschedule all visits – Visits must be scheduled through Activities, and are limited to 2 visitors per resident at a time. These visits are to be limited to 30 minutes, twice a week. Visits are available 7 days a week, during the hours of 11:00am-12:00pm, and 2:00pm-4:00pm. All visitors must be over age 2 and be able to properly wear a facemask.

➢Indoor Visitation – (including end of life, compassionate care & essential caregiver) must be scheduled through Activities or Nursing and are limited to two people per visit.  Visits are to take place in an area designated by Activities. End of Life/Compassionate Care/Essential Caregiver Visitation will be limited to a maximum of 2 hours. Standard indoor visitation is limited to 30 minutes. We encourage POC / Rapid testing prior to visitation in the facility. Visits will take place under observation of a facility employee.

  • Indoor Visitation is now permitted for vaccinated and unvaccinated residents.

➢ Visitors agree to pre-visit screening (including temperature being taken) and an informed consent must be signed by both the visitor and the resident in writing that they are aware of the possible dangers of exposure to COVID-19.  Visitor will be provided instruction on hand hygiene.

➢ Visitors and resident agree to wear face coverings & to have no physical contact unless resident and visitors are fully vaccinated. Face coverings are still required for fully vaccinated residents and visitors. Adherence to the CDC guidelines for 6-foot social distancing is mandatory unless resident and visitors are fully vaccinated. Outdoor visits will take place on the back patio under observation of a facility employee. Smoking is not permitted during visits.

➢ Visitors / resident understand that visits may be cancelled or rescheduled on short notice due to unforeseen circumstances (i.e. inclement weather or resident feeling ill).

➢ Food or gifts during visit will not be allowed to be given to a resident during the visit- Visitors may bring items for the resident but must leave the package at the front reception desk or as directed by the facility. Smoking is not permitted during visits.

➢ Visits are contingent on compliance with all safety protocols.

➢ Visitors have no access to other areas within the facility (including bathrooms).  Please prepare accordingly.
